We are seeking a compassionate and dedicated Hospice Medical Social Worker to join our team, providing invaluable support to patients and their families during one of life's most challenging times. In this critical role, the Hospice Medical Social Worker will offer counseling, emotional support, and practical assistance to ensure that patients facing terminal illnesses can maintain their dignity and quality of life. This position requires a deep understanding of the medical and emotional needs of patients and their families, as well as the ability to navigate complex healthcare systems. Our ideal candidate will work closely with an interdisciplinary team, including physicians, nurses, and other healthcare professionals, to create and implement individualized care plans tailored to the unique needs of each patient. The Hospice Medical Social Worker will also facilitate communication between family members and healthcare providers, helping to mediate conflicts and address any concerns. Through active listening and empathetic engagement, the social worker will help patients and their families cope with grief, loss, and the emotional strains associated with end-of-life care. Other duties:
- They work closely with medical teams to coordinate treatments, services, and resources that can help improve a patient’s quality of life.
- They provide a wide range of supports, based on the specific needs of the patient and the patient's family. They advocate, problem solve, listen, and connect people to resources.
- They also provide resources such as counseling and referral services.
- They help families cope with the stress of dealing with a terminal illness and coordinate care for those who may be facing financial or other hardships due to the illness.
Qualifications:
- Master's Degree in Social Work
- Must have experience as a social worker
- Must have reliable transportation
