About the Role: My client, a growing NDIS Registered service provider, is seeking a dedicated RN Care Manager. You will ensure smooth operations and high-quality care for our participants.
Key Responsibilities:
- Conduct client assessments and develop care plans.
- Coordinate medical appointments and community resources.
- Advocate for clients' rights and quality care.
- Communicate effectively with clients, families, and healthcare professionals.
- Mentor and coach junior nursing staff.
Skills & Experience:
- Registered Nurse with current AHPRA Registration.
- Experience in care coordination or case management.
- Strong people management and organisational skills.
- Excellent communication and interpersonal skills.
- Knowledge of healthcare regulations and standards.
- Proficiency in MS Office and CRM systems.
- Current NDIS Worker Screening and Working with Children Check or willingness to obtain.

Qualifications:
- Registered Nurse Qualification with current AHPRA Registration
- Proven experience as a care leader within the health, community or disability industry
- People management skills including performance management, delegation, follow-up and goal setting with direct reports
- Previous experience in care coordination, case management, or a related role, preferably in a healthcare or social services setting.
- Excellent interpersonal and communication skills, with the ability to build rapport and collaborate effectively with diverse individuals and teams.
- Strong organisational skills, attention to detail, and the ability to manage multiple tasks and priorities.
- Empathetic, patient, and compassionate approach when working with clients and their families.
- Knowledge of relevant regulations and standards related to healthcare and social services.
- Proficiency in MS Office (MS Word, Excel and PowerPoint)
- Previous use of CRM systems
- Excellent time management skills and the ability to prioritize work
- Current NDIS Worker Screening and Working with Children or willing to obtain (essential)
- Knowledge of workplace health and safety legislation requirements
